Initial Comments:


Based on the findings of an off-site unannounced follow-up survey completed 7/21/2023, Integrity Home Care LLC had corrected the deficiency cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51. The deficiency was cited as a result of a State relicensure survey completed 9/3/2021.

611.52(d) LICENSURE
Proof of Residency

Name - Component - 00
The home care agency or home care registry may request an individual required to submit or obtain a criminal history record to furnish proof of residency through submission of any one of the following documents:
(1) Motor vehicle records, such as a valid driver ' s license or a State-issued identification.
(2) Housing records, such as mortgage records or rent receipts.
(3) Public utility records and receipts, such as electric bills.
(4) Local tax records.
(5) A completed and signed, Federal, State or local income tax return with the applicant ' s name and address preprinted on it.
(6) Employment records, including records of unemployment compensation

Observations:


Based on a review of agency job description, personnel files (PFs) and staff (EMP) correspondence it was determined the agency failed to verify proof of residency in this Commonwealth for the 2 years preceding the date of hire (DOH) for one (1) of two (2) PFs reviewed (PF2).

Findings included:

A review of the agency documents on 7/12/2023 at approximately 3:00 PM revealed, "Personal Care Aide Job Description...Knowledge, Skills & Abilities Required...*Pass state required criminal and abuse background checks where required. *Verification of employment eligibility (I-9) in accordance with local, state, and federal law and regulations. *Able to provide proof of valid driver's license, if applicable..."

A review of PF #2 was conducted on 7/12/2023 at approximately 2:40 PM, the employee's date of hire (DOH) was 2/14/2022. Documentation within the file confirmed a Pennsylvania driver's license with an issue date of 7/12/2021 was provided. Also within the PF was an utility bill with a date hand written "Feb 2022". There was no additional documentation in PF to confirm the agency verified proof of residency from the list of required documents in the Commonwealth for two (2) years immediately preceding the date of hire.

Per phone conversation with EMP2 human resources on 7/14/2023 at approximately 1:28 PM no additional proof of residency was available.













Plan of Correction:

How will the Agency resolve this deficiency?
The Agency immediately requested that the noted existing caregiver, locate and return to the Agency proof of residency within the State of Pennsylvania for two years or longer. He will be given 15 calendar days
to provide evidence that satisfies the regulation 611.52(d). The Agency will also review all personnel files to ensure that each employee satisfies the residency requirements.

What will you do to ensure this does not happen again?
The Agency will review the regulations related to its State residency requirements (611.52.d) to make sure they are totally competent in understanding what types of valid evidence
is required to satisfy proof of residency for at least a two year period.

What process will be put in place to make sure this is regularly monitored?

The Agency has a Continual Quality Improvement Program in place that requires us to consistently review policies, procedures, forms and processes. We will
add Employee Proof of Residency as a Quality Indicator to be monitored monthly with an objective of reaching 100% compliance. Quarterly reports will be developeded
to document the results of our internal audits and corrective actions will be established if our objective is missed.
